Before participating in any diving or freediving courses or programs, all student divers must complete paperwork, such as a brief scuba medical questionnaire that ask about medical conditions that could be a problem while diving.

NAUI Waiver

This Diver Medical Participant Questionnaire provides a basis to determine if you should seek out a physician's evaluation and approval to participate in scuba diving or freediving. If you have any concerns about your diving fitness not represented on this form, consult with your physician before diving. For your safety, and that of others who may dive with you, answer all questions honestly.
